Video not playing (either on PlexWeb or iOS Devices)

I spent the past couple days trying to resolve my issue. I have tried looking thru the forum to see if I could find some answer but nothing seem to work.

The issue: Either on PlexWeb on the Mac or on any iOS Device (iPad or ATV4), I can see all my files in the directory, but when I select one to play, it showed a blur screen for a second then back to the description page. I've tried this on several videos, same results. The issue mainly occurs with MP4 files (I have some MKV which are able to play if Direct Play/Direct Streaming are off, but about 85-90% of my library consists of MP4. I also have SRT files).

I've seen people mentioned about Direct Play and Direct Streaming and I've tried to play with it, same results. I have even tried playing with some other options just because, still nothing. I have tried to delete the library and created a new one. Tried to remove PMS and reinstalled it on the Mac. The same issue still persist.

Specs:
OS: El Capitain (10.11)
PMS: 0.9.14.6.1620-e0b7243
iOS: 9.2 (iPad), 9.1 (ATV4)

Attached is the Log File from the iPad. Strangely enough, there is no log file for Plex on my Mac. There is a Logs folder, but consisting of other stuff, nothing related to Plex. (Yes, the option to enable debug logging is on for the server). There is a Temp folder as well with nothing in it (for the Transcoder temporary directory).

Ideas?
