Remote access Not available outside your network...but it is...sometimes???

This is a repost. I'm sorry. I didn't realize I was posting in "Android mobile" when I posted it. It won't happen again...

What I have - Time Warner cable internet, Motorola SURFboard SBG6580 (purchased 3.5 years ago), PMS is run off a Mac OS X El Capitan v10.11.6. I have 6 "friends" on my PMS all in different states. No I'm not port forwarding, yes I enabled UPnP.
I've read a lot of posts on a couple different forums and haven't come across anything that's the exact same as my situation. I'm not necessarily looking to get step by step instructions on how to fix my remote access (yet), but more to start me in the right direction. More directly is it a router issue or a PLEX glitch in the matrix?
My situation. I recently relocated to a different state. In the state I left I had Verizon FiOS using their router/modem combo. PLEX worked like a champ. I moved, got TWC and knocked the dust off the Motorola router mentioned above. At first my network was not able to be seen out of my network at all, as expected based on the "not available..." message. One day I got a call from one of my "friends" saying he was able to see my server and access items, however, it would only let him access certain things with no rhyme or reason to which he could/couldn't see. Also, he could only see my server on his Mac. None of his other devices could see it. My other friends had issues ranging from couldn't see it at all to could see my server but couldn't access anything. So here's my questions:
Does this sound like a router issue - for example because it's 3.5 years old and/or it could have firmware/software issues not allowing PLEX to be run freely or locked ports?
OR
Is it a PLEX issue/glitch in the matrix? I've read a lot of issues with the latest updates. If PLEX is saying my server is not available outside of my network, how are my friends able to see and use (select) files? I'd think it would be an all or nothing kind of thing when working with 1's and 0's...
I'm not super savvy when it comes to the more advanced stuff with networking. I have read a lot, on PLEX support and forums, so I have an idea of what I can do to troubleshoot (and so I'm not wasting your time by repeating questions). All of my theory's have come from what I've read on the forums. I'm hoping it is a simple fix, like my router is old or it's a glitch with PLEX so I thought I'd ask the group before I lock myself in a room for a day trying to learn how to port forward.
I'm trying to start big picture here. Router or PLEX or something I haven't read about. Things I read I could try but wanted to wait in case it's simple - port forward and erase my PMS and start over. I've already done the little tricks like make sure my UPnP was enabled, clicking disable remote access and clicking retry to map the server then enabling and retrying again, restarting computers and modems and anything else I could restart etc. I know this is popular topic and it comes up a lot so thanks in advance for taking the time to help!
Last thing if it is a router issue what do you recommend? I have cable internet so I'd prefer an all in one type.
